I agree to the Mighty Kids Creative Arts LLC participation waiver and release of liability.  I understand that Mighty Kids Creative Arts LLC is an activity based studio and injuries may occur.  By checking the Liability Waiver box I agree to not hold Mighty Kids Creative Arts LLC liable for injuries sustained during normal program activities.
